Woman’s Club of Bethesda would like your permission to use images and video taken of you to showcase the club on our website, Facebook and Instagram pages.

Please provide emergency contact information of at least two individuals we may contact if/when necessary. Indicate the relationship to you; i.e. daughter, son, sister, friend, neighbor, etc. Give phone number/s (cell, preferably) and indicate the area in which the contact persons lives. Address not necessary, just area, such as: Bethesda, Rockville, Silver Spring, Olney, Laurel, Virginia, etc.  This is confidential information, intended only for emergency use by members of the Woman’s Club of Bethesda.
